본문 : Homonymous hemianopic deficits may occur with any lesion of the retrochiasmal visual pathways. Ulegyria is a specific type of cortical scarring in the deep regions of the sulcus that leads to distortion of the gyri. We present a patient with homonymous quadrantanopsia caused by occipital lobe ulegyria. A 23-year-female was referred to our clinic because of the visual field defect incidentally discovered during the preoperative evaluation for refractive surgery at another clinic. But, she did not have any subjective symptoms before. The automated perimetry demonstrated the right lower homonymous quadrantanopsia with macular sparing. She had no any other systemic diseases. Visual acuity was 20/20 in both eyes, and the refractive error was -5.75 + 1.00 D x 90 in the right eye and -5.25 + 1.00 D x 90 in the left eye. The pupil showed normal response to light and near stimulation in both eyes. The extraocular examination showed orthotropia at primary gaze without any limitation. The fundus examination showed myopic appearance of optic disc. The magnetic resonance imaging (MRI) revealed areas of ulegyria involving left occipital lobe consistent with visual field defect. |
