University Malaya Specialist Center
Suet Mei Chan , Angela Voon Pei Loo
Purpose : To report the use of aniridia implants in four patients with history acute glaucoma attackand ocular blunt trauma . Methods : A case series of 4 patients requiring aniridia implants with history of acute glaucoma and blunt ocular trauma to improve visual acuity ( VA) and to control symptoms (reduce glaring and haloes ). Results : Mr T, 74 years old, presented with history of shuttle cock injury to his right eye and sustained right traumatic mydriasis and cataract. Right eye phacoemulsification with posterior chamber intraocular lens and Morcher aniridia implant ( 50 F x 2 units) insertion were performed 3 weeks later. Postoperatively, patient experienced less photophobia and has better shape of right pupil. Mr. L , 44 years old who sustained shuttle cock injury to his right eye.Post trauma patient was complaining of glare and reduced visual acuity. Best corrected vision was 6/36. On examination, traumatic mydriasis and posterior subcapsular cataract were noted. Right eye phacoemulsification with PCIOL and Morcher aniridia implants (50F x 2units). Postoperatively patient had regained satisfactory vision with less glaring. Mr P, 60 years old with known left eye pupillary block glaucoma secondary to subluxated lens with hand movement vision. He had undergone left phacoemulsification , vitrectomy , Morcher aniridia implant 67G uneventfully. Vision was improved significantly with satisfactory anterior segment reconstruction. Madam C, 74 years old, both eyes glaucoma for more than 18 years. She had both eye peripheral iridectomy. Both eye vision 6/9 with enlarged pupils. Right phacoemulsification with PCIOL and Morcher ring ( 50Fx 2 units)implanted. Glaring was significantly reduced. Conclusion : Morcher aniridia implants are increasingly popular choice for surgeons in patients with damaged iris which may distort the pupil causing severe functional disability from glare and haloes, and hence to improve visual acuity, as well as to reconstruct the anterior segment of patients with aniridia.
