본문 : Purpose: The nature and source of the fluid in optic disc pit(ODP)-associated maculopathy, a rare congenital disease of the optic nerve head, remains controversial. We report our experience using a continuous intraoperative optical coherence tomography(OCT)-enabled microscope system for real-time anatomical evaluation of the optic pit during surgery.
Methods: A small-gauge sutureless pars plana vitrectomy was performed with continuous high-definition intraoperative OCT imaging using the Zeiss Rescan 700 device with the Lumera 700 microscope (Carl Zeiss Meditec Inc.) in an 8-year-old girl who presented with symptomatic ODP-maculopathy.
Results: Both pre-operative and intraoperative OCT images did not reveal a channel connecting the liquefied vitreous to the subretinal space, and extensive aspiration immediately near the ODP did not result in drainage of the subretinal fluid. We observed a strongly adherent translucent membrane over the ODP.
Conclusion: Contrary to a previous report, our findings appear to suggest that a different source other than liquefied vitreous may be involved, and that there may be heterogeneity in the pathogenesis of ODP-maculopathy. With this understanding in mind, we believe that surgeons can optimize outcomes by designing a maximal approach that targets the closure of this tract. |
