녹내장안에서 빛간섭단층혈관조영술로 확인한 맥락막 관류이상과 중심시야손상과의 관련성

목적 : To evaluate the frequency of parafoveal visual field (VF) damage in open-angle glaucoma (OAG) eyes with choroidal microvasculature dropout (CMvD) and to assess correlations between the circumference of CMvD and the severity of VF defects 방법 : CMvD circumference and location were determined from choroidal layer segmentation images obtained from optical coherence tomography angiography. Demographic and ocular characteristics, including the presence of parafoveal VF defects, were compared between OAG eyes with CMvDs and those without. Regional VF mean sensitivity (1/L) according to the Garway-Heath mapping method was also compared and logistic regression analyses were performed to derive the clinical factors associated with parafoveal VF defects. The relationships between CMvD circumference and global/regional VF defect severity were 결과 : CMvDs were predominantly found in the inferotemporal region in eyes with βPPA. OAG eyes with CMvDs showed more cardiovascular risk factors, more frequent involvement of parafoveal VF defects, greater glaucomatous VF damage at locations corresponding to the inferotemporal to temporal sectors of circumpapillary vessel density, and greater retinal nerve fiber layer thickness loss compared to those without. Parafoveal VF defects were observed in 96% (42/44) of eyes with CMvDs but only in 24% (9/38) of eyes without CMvDs. A significant association was found between parafoveal VF defects and CMvDs 결론 : Parafoveal VF defect was identified more frequently in OAG eyes with CMvDs than in those without. The involvement of parafoveal VF defects was significantly associated with the presence of CMvD. Among eyes with CMvDs, CMvD circumference had significant correlations with the severity of VF defects. Our findings may provide clinical clues regarding the locality and severity of glaucomatous VF damage in OAG eyes with CMvDs.
