맥락막 미세혈관 결손에 근시 및 녹내장이 미치는 영향

목적 : To investigate the frequency of choroidal microvascular dropout (CMvD) in healthy myopic eyes and compare it with that of myopic open-angle glaucoma (OAG) eyes and to determine whether myopia is independently associated with CMvD. 방법 : Deep-layer microvascular images were obtained using AngioVue optical coherence tomography angiography (OCT-A) in 89 myopic OAG patients and 89 healthy myopic subjects. The CMvD was defined as a complete loss of the microvascular network on choroidal layer vessel density map. Frequency of CMvD was compared between myopic OAG and healthy myopic eyes. Hemispheric comparisons were performed between CMvD-affected and -unaffected hemiretina. The association of myopia with the presence and circumferential size of CMvD was evaluated using logistic and linear regression analyses. 결과 : The CMvD was identified in 40 eyes (47.6%) of myopic OAG, whereas no CMvD was found in healthy myopia. Mean hemispheric VF sensitivity (294.9±321.1 vs. 610.5±387.5, p=0.006), RNFL thickness (68.9±11.5 vs. 82.5±20.6µm, p=0.001), and VD (50.5±7.3 vs. 53.9±8.8%, p=0.048) were significantly worse in the CMvD-affected hemisphere than in the CMvD-unaffected hemisphere. The presence and circumferential size of CMvD was significantly associated with parameters (VF MD, RNFL thickness, and VD) representing severity of glaucoma rather than parameters (RE and AL) representing myopic change. 결론 : The CMvD was observed only in glaucomatous eyes and associated the severity of glaucomatous damage in myopic subjects. The CMvD may be the pathognomonic finding of glaucomatous damage not myopic change
