목적 : To describe a distinctive photoreceptor layer (PRL) disruption as seen by spectral-domain optical coherence tomography (SD OCT) in eyes with pachychoroid.
방법 : We present 21 patients (22 eyes) with PRL disruption in association with pachychoroid. PRL disruption was defined as Decrease in reflectivity of EZ/IZ band with focal discontinuation/break. Patients were evaluated by comprehensive ophthalmic examination, fundus photography and dye angiography. SD OCT was used to analyze the PRL, outer retinal architecture, and choroid.
결과 : All 27 lesions demonstrated abnormalities in RPE layer (RPE atrophy in 22 and irregular RPE undulation in 8 lesions). Accompanying features were hyperreflective foci in 8, focal thinning of ONL with enlarged OPL in 15, subsidence of the OPL/INL in 11, hyporeflective wedge-shaped band within the OPL in 15 lesions. No subjects had a history of or signs suggestive of antecedent subretinal fluid. In 4 eyes, drusenoid lesions were seen at the site of PRL disruption prior to the baseline examination. Most eyes did not show morphologic change during a mean follow-up of 20.3 months.
결론 : Disruption in the PRL with focal RPE and outer retinal abnormalities can be present in pachychoroid, and should be considered as an atrophic form of pachychoroid pigment epitheliopathy.
