시신경연접부 맥락막 두께와 빛간섭단층혈관조영술로 관찰한 시신경주위 깊은 층의 모세혈관결손과의 지형학적 연관성

목적 : To investigate the topographic relationship between the parapapillary deep–layer microvasculature dropout (MvD) as assessed by optical coherence tomography (OCT) angiography (OCTA) and the juxtapapillary choroidal thickness (JPCT) in primary open-angle glaucoma (POAG). 방법 : Peripapillary circulation was evaluated using swept-source OCTA in 138 POAG eyes and 56 healthy control eyes. The 138 POAG eyes consisted of 69 eyes with MvD and 69 eyes without MvD that were matched for age, axial length and visual field mean deviation. The topographic correlation between the MvD and the JPCT was determined in the POAG group. JPCT was compared between POAG eyes with and without MvD, separately in subgroups divided according to the type of parapapillary atrophy (group with only β-zone, n=70 and group with mixed β and γ-zones, n=68). 결과 : None of healthy control group showed MvD. The JPCT was significantly thinner in POAG group than in healthy control group in 6 and 7 o’clock locations(all P=0.003). In POAG eyes with MvD, the distribution of thinner JPCT corresponded to the frequency distribution of MvDs. The JPCT did not differ between POAG eyes with MvD and those without when the MvD was associated with β-zone. However, when MvD was associated with mixed β and γ-zones, POAG eyes with MvD had significantly thinner JPCT than those without in 6 and 7 o’clock locations(all P<0.0042), where the MvD presented most frequently. 결론 : Thinner JPCT was topographically correlated with the location of MvD, specifically in eyes with mixed β and γ-zones but not in those with β-zone only. The mechanism and pathogenic significance of the MvD in POAG with differed type of parapapillary atrophy remain to be determined.
