목적 : To classify eyes with angle closure (primary angle closure (PAC) + primary angle closure glaucoma (PACG)) in terms of features provided by both anterior segment optical coherence tomography (AS OCT) and ultrasound biomicroscopic (UBM) images.
방법 : Eyes with angle closure were imaged using both AS OCT and UBM. Anterior chamber depth (ACD), anterior chamber width (ACW), iris cross-sectional area (IA), iris thickness at 750 µm from the scleral spur (IT750), iris curvature (IC), lens vault (LV), and anterior chamber area (ACA) were determined by AS OCT image, and trabecular-ciliary process distance (TCPD), trabecular-ciliary process angle (TCA) and ciliary body thickness (CBT1) were estimated by UBM image. Two kinds of hierarchical cluster analyses were performed, using 1) AS OCT parameters only and 2) both AS OCT and UBM parameters.
결과 : Total 90 eyes of 90 patients were enrolled. Two clusters according to AS OCT only showed significant differences related with most of AS OCT parameters (ACA, ACD, IT750, ACW, LV, IA, AOD500) and PD. Two clusters according to AS OCT with UBM showed significant differences related with most of UBM parameters (TCA, TCPD), mean f/u IOP, IA, AOD500, and PD. But, ACA, ACD, IT750, IC, ACW, and LV were similar between two clusters divided by AS OCT and UBM. When only UBM was used in clustering analysis, the results were same with those of both AS OCT and UBM.
결론 : Eyes with angle closure can be clustered distinctly according to shape of structures behind the iris with the aid of UBM. A cluster of eyes with lower TCA and TCPD, similar features with plateau iris configuration, was created by UBM. When two imaging modalities were used simultaneously, UBM had more effect on clustering eyes with angle closure compared with AS OCT. UBM can give additional clues to differentiate angle closure in detail.
