녹내장 환자에서 분절화된 황반부 망막층의 임상적 분석

목적 : To measure the thicknesses of retinal layers in the macular area and evaluate structure and function in glaucoma. 방법 : This study investigated 84 eyes with early, moderate, or advanced glaucoma. Retinal nerve fiber layer (RNFL), ganglion cell layer (GCL), inner plexiform layer (IPL), outer plexiform layer (OPL), inner nuclear layer (INL), and outer nuclear layer (ONL) thicknesses were measured using the Spectralis spectral-domain optical coherence tomography (SD-OCT) using automated segmentation software. The corresponding mean sensitivity (MS) values were measured using standard automated perimetry (SAP) for functional evaluation. 결과 : RNFL, GCL, and IPL thicknesses reduced significantly, while INL thickness increased, as glaucoma progressed. The ratio of the inferior/superior thickness of the RNFL, GCL, and IPL and the corresponding MS value for the VF (visual field) were positively, and that of the inferior/superior thickness of the INL and ONL were negatively correlated with the MS value of the VF for the corresponding area. Hypertension, RNFL, GCL, and IPL thicknesses, VF mean deviation, and disc hemorrhage were identified as significant factors in INL thickening. Multivariate logistic regression analysis implicated IPL 결론 : Macular retinal structure and functional losses correlate with decreased inner retinal layer and with increased INL and ONL thicknesses in glaucoma.
