녹내장 환자에서 Slow-sequence 다초점 망막 전위도 검사의 진단력

목적 : To evaluate diagnostic performance of multifocal electroretinograms(mfERGs) elicited by low-frequency stimuli in patients with glaucoma. 방법 : A slow sequence mfERG was recorded in 52 eyes of 36 normal, 32 eyes of 25 early glaucoma , 32 eyes of 24 advanced glaucoma subjects. The stimulus frequency was 6.25 Hz, and the stimulus was a circle which radius of the inner border was 6.8˚ and that of the outer border was 20˚, placed in macular region. We measured the response density of a negative wave(N1) followed by a positive wave(P1), and followed by a slow negative wave(N2) at all 3 retinal areas including center, superior and inferior half annulus and compared diagnostic performance of above measurements. 결과 : The N2 amplitude and N2/P1 amplitude ratio which were assessed at all 3 retinal areas was significantly low in glaucoma subjects compared to normal subjects(p<0.05, respectively). The area under the ROC curves of N2 amplitude and N2/P1 amplitude ratio in center area were 0.77 and 0.73, and those of value in superior and inferior half annulus were 0.73/ 0.67 and 0.79/ 0.75, respectively. The AUCs for N2 amplitude tended to be higher than the AUCs for N1, P1 amplitude in all retinal areas, and those of center area was not significantly different from those of other retinal areas. 결론 : The N2 component of the slow-sequence mfERGs was reduced in patients with glaucoma compared to that of normal controls. Assessment of N2 component in slow sequence mfERG is useful for detecting glaucomatous damage like as Photopic negative response(PhNR).
