목적 : To investigate the risk factors associated with multiple retinal nerve fiber layer (RNFL) defects
방법 : Data from subjects with multiple RNFL defects and glaucoma patients without multiple RNFL defects was analyzed. The term multiple RNFL defects describes three or more isolated defects separated by a comparatively normal area. Systemic vascular risk factors associated with multiple RNFL defects were analyzed. Optic disc parameters and RNFL thickness were measured using spectral domain optical coherence tomography. Posterior pole profiles were evaluated using fundus photographs. The presence of parafoveal visual field points depressed < 5% on pattern deviation plots was analyzed.
결과 : Patients with multiple RNFL defects showed a higher proportion of hypertension, end-stage renal disease, and cerebrovascular disease than those without multiple RNFL defects (all P<0.05). Eyes with multiple RNFL defects were associated with lower cup-to-disc ratio, lesser optic disc torsion, and smaller PPA area compared to glaucoma patients without multiple defects. The number of patients with parafoveal visual field points depressed < 5% on pattern deviation plots was higher in subjects with multiple RNFL defects than in those without multiple RNFL defects (P=0.048).
결론 : The presence of multiple RNFL defects had relevance to systemic vascular risk factors and higher risk of parafoveal scotoma. Clinicians should be aware of patients with multiple RNFL defects to avoid missing systemic vascular disease.
