목적 : To study vascular density (VD) change according to glaucoma severity assessed by optical coherence tomography angiography (OCT-A, Cirrus HD-OCT 5000 system with Angioplex OCT angiography; Carl Zeiss Meditec) and compare diagnostic accuracy between OCT-A and optical coherence tomography (OCT) in healthy and glaucoma groups.
방법 : Total 197 eyes of 100 healthy, 60 early, 37 moderate to advanced glaucoma patients were included. All subjects underwent OCT-A and OCT tests, along with visual field (VF) tests. The relationship between OCT-A and glaucoma severity and comparison of diagnostic accuracy between OCT-A and OCT at various glaucoma stage were analyzed.
결과 : The Area under the receiver operating characteristic curves (AUROC) was highest for peripapillary(pp) average RNFLT(0.890), followed by ppVD at 7 o’clock(0.878), inferotemporal GCIPLT(0.834) and macular superotemporal VD(0.616) for differentiating healthy and early groups. Between healthy and moderate to advanced groups, inferior quadrant ppVD(0.996) was the highest, followed by inferotemporal GCIPLT(0.991), ppRNFLT at 7 o’clock(0.989), and macular inferotemporal VD(0.783). In pairwise comparison ROC curves, significant differences were found only between GCIPLT and VD at macular area.
결론 : OCT-A demonstrated reduced VD in glaucoma eyes compared to healthy eyes and the degree of decrement was dependent on the severity of glaucoma. The diagnostic accuracy was comparable between OCT-A and OCT in the peripapillary area, but OCT was superior to OCT-A in the macular area. This may result from smaller scan area of OCT-A compared to OCT in the macula.
