백내장 수술 후 발생한 급성 안내염: 164 사례분 석 및 시력예후인자

목적 : To evaluate the clinical characteristics, bacterial culture and factors affecting final visual outcome by analyzing patients referred with postoperative endophthalmitis after cataract surgery. 방법 : We retrospectively reviewed the medical records of 164 patients referred with endophthalmitis following cataract surgery in single center between January 2008 and December 2015. We performed primary vitrectomy to all eyes with initial visual acuity of light perception. Eyes with visual acuity of hand motions or better underwent either intraocular antibiotics injection or primary vitrectomy. To evaluate factors affecting final visual acuity, initial visual acuity, and time from surgery to symptoms of endophthalmitis, types of treatment, presence of hypopyon and culture results were investigated 결과 : Of the 164 patients, visual acuities at presentation were hand motions or less in 111 patients (67.7%) and final visual acuities after treatments were 0.5 or better in 93 patients (56.7%). Cases with initial visual acuity of hand motions or better achieved favorable outcomes whereas cases with gram-negative infection or endophthalmitis occurring within 3 days postoperatively showed poor prognosis. The rate of reintervention of primary vitrectomy was significantly higher than primary IOAI (P<0.05). 결론 : Visual acuities at presentation, type of cultured organism and onset of endophthalmitis after cataract surgery are significantly related to good visual prognosis. Final visual outcome of initial vitrectomy versus intraocular antibiotic injections were unclear. However, the rate of reintervention is lower with primary vitrectomy.
