목적 : To evaluate the risk factor for rapid progression to proliferative diabetic retinopathy (PDR) in patients with type 2 diabetes mellitus.
방법 : The study included 118 patients with severe non-proliferative diabetic retinopathy (NPDR) divided into two groups according to the rate of retinopathy progression: a rapidly progressive group (progression to PDR within 12 months; n = 60); a control group (no progression; n = 58). All patients underwent detailed ophthalmic and systemic examinations. Data on medical history and health behaviors were collected.
결과 : Patients in the rapidly progressive group were younger than those in the control group (49.0 ± 10.4 vs. 62.9 ± 10.2 years, P < 0.001). A shorter duration of diabetes and an increase in HbA1c, fasting blood sugar, total cholesterol, and low-density lipoprotein cholesterol (LDL-C) levels were associated with rapid progression to PDR. Multivariate analyses showed that age (adjusted odds ratio [aOR] 0.862, 95% CI 0.80−0.93) and a high LDL-C level (>100 mg/dl) (aOR 4.852, 95% CI 1.16−20.26) were independently related to rapid progression after adjustment for confounding factors.
결론 : Our study suggests that age and a high LDL-C level may affect the rapid progression to PDR in type 2 diabetic patients with severe NPDR.
