비문증 환자의 정신심리분석

목적 : In this cross sectional study, we evaluated the degree of psychologic distress in patients with symptomatic vitreous floaters. 방법 : We enrolled 61 symptomatic vitreous floaters patients and 34 control. We evaluated the degree of floater associated discomfort (mild, moderate or severe) and also assessed the level of depression, stress, and anxiety using self-reporting questionnaire. We evaluated posterior vitreous status using optical coherence tomography. We identified the factors associated with symptomatic floaters and complete posterior vitreous detachment using multiple logistic regression analysis, and performed multiple linear regression analysis to identify factors associated with the degree of floater discomfort. 결과 : On multivariate analysis, complete posterior vitreous detachment (PVD), depression and younger age were significantly associated with symptomatic floaters. However, only older age was significantly associated with the prevalence of complete PVD. Of 61 symptomatic vitreous floater patients, 20 reported mild, 24 reported moderate, and 17 reported severe for their discomfort level. The degree of floater discomfort was well correlated with the level of psychological distress, especially with the stress level (F(1,58)=25.68, R2=0.307, R2Adjusted=0.295, Beta=0.065, p<0.001). 결론 : Symptomatic vitreous floater patients showed higher level of psychologic distress such as depression, perceived stress and anxiety. The prevalence of symptomatic vitreous floaters was associated with PVD, which is associated with aging process and depression level and the degree of discomfort associated with vitreous floaters was significantly associated with psychologic distress, especially perceived stress level. These suggests that psychologic distress is prevalent in symptomatic vitreous floater patients and their visual discomfort is highly associated with their psychologic problems.
