목적 : The aim of the present study was to investigate the postoperative higher-order aberrations (HOAs) after implantation of aspheric intraocular lenses (IOLs) in eyes which underwent refractive surgery.
방법 : This retrospective case review included 70 eyes of 55 patients. Thirty eyes of 22 patients with previous history of LASIK or PRK for myopia underwent cataract surgery with implantation of aspheric IOLs (Tecnis ZCB00) which have negative spherical aberration. The remaining 40 eyes of 33 patients only underwent cataract surgery with the same IOLs implantation. Automated keratometry, manifest refraction, corneal topography and aberrometry were conducted preoperatively and 1 month postoperatively. Surgically induced astigmatism (SIA) and prediction error of IOL power were calculated.
결과 : SIA calculated by automated keratometry value showed significant difference between two groups (p=0.020). However, there was no difference in SIA calculated from corneal topography (p=0.451). After cataract surgery, corneal spherical aberration (p=0.001) and ocular spherical aberration (p=0.001) are significantly higher in refractive surgery group than in non-refractive surgery group. The internal spherical aberration of both groups showed negative value but difference was not significant (p=0.272).
결론 : Refractive surgery group showed higher corneal spherical aberration compared to non-refractive surgery group. In non-refractive surgery group, ocular spherical aberration was effectively reduced by implantation of aspheric IOLs. However, in refractive surgery group, ocular spherical aberration value was significantly large due to partial correction of corneal spherical aberration by aspheric IOLs.
