시신경유두이상의 진단을 위해 아이폰 카메라로 촬영한 안저사진의 진단적 유효성 분석

목적 : The purpose of this study is to evaluate the relative diagnostic accuracy of detecting optic nerve head (ONH) abnormalities in photographs taken by the iPhone4S camera attached to the PanOptic Ophthalmoscope (ICPO) vs. those obtained with standard fundus camera photography. 방법 : New patients over 18 years of age referred to the MEEI Neuro-ophthalmology service for acute vision loss or headache were enrolled in this study, which was approved by the local IRB. Fundus photographs were taken of all patients before and after pupil dilation with ICPO and post-dilation with a standard Topcon fundus camera. The photographs were evaluated by two neuro-ophthalmologists who were masked to the clinical impression and the reason for referral. 결과 : Thirty-four patients (68 eyes) were enrolled. Seventeen patients were clinically diagnosed with ONH edema and 11 patients with ONH pallor. When reviewing photographs, the mean values of sensitivities and specificities of diagnosing ONH edema was 69.8% and 96.6% (undilated ICPO), 69.7% and 94.3% (dilated ICPO), and 70.2% and 94.4% for dilated Topcon photography, respectively. The mean values of sensitivities and specificities of diagnosing ONH pallor was 39.5% and 90.9% (undilated ICPO), 56.1% and 79.9% (dilated ICPO), and 68.2% and 92.3% for dilated Topcon photography. 결론 : The sensitivity and specificity to detect ONH abnormalities was similar regardless of technique used to obtain photographs. Both techniques have moderate sensitivity and high specificity to determine the presence or absence of ONH edema. Both techniques are specific but have a very low sensitivity to detect the presence of ONH pallor.
