갑상선안병증에서 TNF-like protein 1A(TL1A)의 활동성 지표로서의 의미

목적 : To assess the serum TL1A concentrations and TL1A mRNA expressions in orbital fat tissue of Graves’ orbitopathy (GO) patients. 방법 : Orbital fat tissues from 11 GO patients and 7 control subjects and sera from 34 GO patients and 11 normal controls were included. Concentrations of TL1A were measured from sera using enzyme-linked immunosorbent assay, and mRNA expressions of TL1A were evaluated from orbital fat tissue. Demographics, clinical activity score (CAS), severity (NOSPECS) score and biochemical parameters for GO were obtained. We compared the serum TL1A levels and expression of TL1A in orbital fat tissue between GO patients and control subjects. Also, relationships between clinical factors and TL1A were analysed. 결과 : Expressions of TL1A mRNA in orbital fat tissue were increased up to 8.7 folds in GO patients compared to normal patients (p<0.01). Serum TL1A levels were elevated significantly in GO patients especially who were in active inflammatory phase compared to patients who were in normal or inactive phase. (p=0.04 and p<0.01). Thyrotropin-binding inhibitor immunoglobulin (TBII) have positive correlation with serum TL1A levels (r=0.486, p<0.01). The cutoff value for the active GO of serum TL1A was 100.36pg/mL with sensitivity of 61.5% and specificity of 61.9%. The AUC was 0.769 (95% CI : 0.612-0.926). 결론 : TL1A may be useful as an ancillary inflammatory activity marker of Graves's orbitopathy.
