외전신경마비에서 내직근 후전술과 병합한 상직근 전위술의 효과;수직근 전위술과 비교

목적 : To compare effectiveness of superior rectus transposition/medial rectus recession (SRT/MR) vs. inferior and superior rectus transposition (VRT) for acquired sixth nerve palsy. 방법 : The medical records of a consecutive series of patients with an acquired sixth nerve palsy who underwent VRT (1988-2005) or SRT/MR (2013-16) by a single surgeon were reviewed. The pre and postoperative findings were compared between the two groups. 결과 : Eight patients underwent SRT/MR and 8 patients underwent VRT. Lateral fixation was performed on all but 4 patients in VRT group. Median follow-up was 6.2 months in SRT/MR group and 17.3 months in VRT group. Preoperative esotropia and abduction deficit were similar in both groups (41.9 vs 53.1 PD and -4.7 vs -4.5; p=0.38). No additional procedures were performed in SRT/MR group but not in VRT group (SRT/MR, 0; VRT, 1.3±1.2; p<0.001). At last follow-up, esotropia and abduction were similar in both groups(7.1 vs 8.5 PD; p=0.721). There were no new persistent vertical deviations. 결론 : Final outcome was similar with SRT/MR vs. VRT. However, fewer additional surgical procedures were needed with SRT/MR.
