0.5% 아프라클로니딘 점안시 안검하수 환자들에서 눈꺼풀각막반사간거리 변화가 다양한 이유

목적 : The variable responses in MRD1(marginal reflex distance) are showed in blepharoptosis after instillation of topical 0.5% apraclonidine. We thought the different concentration of the adrenergic receptors of Muller muscle might be play a significant role in this phenomenon. 방법 : This study included 28 eyelids of 24 patients who underwent blepharoptosis surgery at Kim’s Eye Hospital from June 2014 to October 2015. We preoperatively measured the MRD1(marginal reflex distance) before and after instillation of topical 0.5% apraclonidine and divided 2 groups according to response of topical 0.5% apraclonidine, the Group A: increases of MRD1 and Group B: no increases of MRD1. The obtained Muller muscles during the ptosis surgery were stained for α1-D, α2-C, β1 and β2 receptors, and the staining intensity was graded. 결과 : Of 24 patients, 11 patients were female and 13 patients were male. Mean age was 38.5 ±18.8 years. The Group A was 12 eyelids and Group B was 16 eyelids. The α1-D receptor intensity was stronger in Group A than Group B. The α2-C and β1 receptor intensity were stronger in Group B than in Group A. The β2 receptor intensity was not showed significant differences between Group A and B. 결론 : Phenylephrine is a relatively specific stimulant for the α1 receptors, whereas apraclonidine stimulates the α2 receptors. But the Group A (increases of MRD1) had more concentration of α1-D receptor instead of α2-C. The elevating effect of apraclonidine is due to its weak α1 activity.
