목적 : To investigate the effect of maximum amount of two-horizontal muscle surgery in adults with long-standing, large-angle exotropia.
방법 : 10 adults patients with exotropia ≥ 50(△) with no previous surgery were underwent maximum amount of two-muscle surgery (recession:9-11mm, resection 6.5-7.5mm). All had fixed sutures and observed for a minimum 1 year. Postoperative alignment, stereoacuity, and abduction deficit were evaluated.
결과 : Mean age was 41.4years. Successful alignment was achieved in 7 patients (70%) within limit of 10(△) of orthophoria and three required additional surgery including 1 was undercorrected and two were recurred before 2months. Postoperative stereoacuity was improved from 262.2 to 138.9 seconds of arc (p=0.039).
Abduction deficits were seen initially in most patients but in a final visit day for each of patients, they were improved and became asymptomatic.
결론 : Maximum amount of two-muscle surgery with fixed suture are effective in achieving favorable alignments in long-standing large-angle adult exotropia without much abduction deficit. Stereoacuity could be improved through the influence of regaining some fusion ability in these patients.
