목적 : To investigate the effect of intravitreal bevacizumab injection for diabetic macular edema (DME) on the peripapillary retinal nerve fiber layer (RNFL) thickness in patients who underwent the prior panretinal photocoagulation (PRP) therapy.
방법 : We retrospectively reviewed the medical records of the 35 DME patients who underwent the intravitreal bevacizumab injection at least 3 months interval after the PRP. The peripapillary RNFL thickness were measured at baseline, 1 month and 3 month after a bevacizumab injection. To evaluate the intraindividual effect of bevacizumab on RNFL thickness, 1-month or 3-month follow-up OCT data of the eyes that received an intravitreal bevacizumab injection were compared with preinjection data.
결과 : The global RNFL thickness significantly decreased at 1 month (118.34±33.04㎛) after the intravitreal bevacijumab injection at baseline (126.39±44.58㎛) (p=0.001). At 3month after the injection, the global RNFL thickness (117.38±31.59㎛) was also decreased, which was not statistically significant (p=0.097). However, when the eyes which underwent additional injection more than two times during the follow-up period, the global RNFL thickness significantly decreased at 3 month after the initial injection (baseline vs 3 month, 139.86±64.95㎛ vs 120.63±42.21㎛, p=0.097).
결론 : Multiple intravitreal bevacizumab injections for DME in patients who underwent PRP caused a significant decrement in peripapillary RNFL thickness after 1 month and 3 months of follow-up. Cautious interpretation is recommended for evaluation of peripapillary RNFL thickness profiles in patients with DME after intravitreal bevacizumab injection.
