본문 : Purpose: To report a series of patients who experienced unexpected and permanent central visual loss after silicone oil tamponade. Methods: Three patients with central visual loss after silicone oil tamponade were investigated. They underwent comprehensive ocular examinations including fluorescein angiography, optical coherence tomography (OCT), automated perimetry, and electrophysiologic examination. Results: Three men (mean age, 38.7 years) had received surgical repair of rhegmatogenous retinal detachment (RD) with macular attached. Preoperative best-corrected visual acuity (BCVA) was 20/30 or better, but severe visual loss (5 Snellen lines or more) was encountered while silicone oil was in the eyes. There were no other complications associated with RD repair. Final BCVA was not improved after oil removal whereas the retina remained attached in all cases. OCT revealed loss of foveal depression and thinning of inner retinal layers, especially a ganglion cell layer. Electrophysiologic testing showed abnormalities with profound macular dysfunction. Conclusions: Severe visual loss can be occurred after silicone oil tamponade in patients with good visual potential. The visual loss seems to be associated with a significant reduction in inner retinal thickness showing ganglion cell loss in the macula.
