목적 : To evaluate clinical role of pseudohole itself in eyes with epiretinal membrane (ERM) and compared with control group, which have ERM with similar retinal thickness and age.
방법 : This retrospective study included consecutive patients who underwent surgery for idiopathic ERM. ERM and macular pseudohole were diagnosed based on the findings of both fundus photography and OCT. Among the eyes with macular pseudohole, we evaluate the detailed shape of the edge of the pseudohole. The pseudohole cases with the stretched cleavage between the inner and outer retina, or with the thin irregular foveal center were excluded. Control group was selected among the eyes with ERM by comparing juxtafoveal retinal thickness and patients' age.
결과 : Of total 150 eyes, 41 eyes (27.3%) exhibited a steepened foveal pit with epiretinal membrane, and 109 eyes (72.7%) showed epiretinal membrane with loss of foveal pit. Among the 41 eyes with the steepened foveal pit, 20 eyes (48.8%) had the stretched cleavage between the inner and outer retina or the loss of the foveal outer retina and 21 eyes (51.2%) had thick or normal foveal center without stretched cleavage. We evaluated the 21 eyes that had typical pseudohole without cleavage in the OCT images. In addition, we enrolled 21 age and parafoveal retinal thickness-matched patients from among the patients who had an eye with ERM and did not had the pseudohole as a control group. Foveolar thickness of the control group decreased after surgery (504.76±125.14µm to 366.57±64.60µm) but foveolar thickness of the pseudohole group rather increased after surgery (214.05±30.35µm to 292.24±84.40µm). Postoperative visual acuity of the pseudohole group was significantly higher than that of the control group (0.03±0.05 vs 0.07±0.07)(P=0.045). In addition, the time to reach the maximum visual acuity of the pseudohole group was shorter than that of the control group (9.00±9.29months vs 16.96±14.38months)(P=0.033).
결론 : The mean visual acuity of the pseudohole group was better than that of the control group. Foveal anatomic characters may reflect better surgical outcomes. Pseudohole itself can be considered as a good prognostic factor in eyes with ERM if there were few secondary changes surrounding pseudohole.
