목적 : To determine the difference of pupillary functions in the acute phase of optic neuritis (ON), nonarteritic anterior ischemic optic neuropathy (NAION) and Leber`s hereditary optic neuropathy (LHON). 방법 : We conducted a retrospective chart review of 76 patients who were diagnosed with unilateral ON, NAION, LHON, and 77 age-matched controls between 2011 and 2016. Pupillary responses were measured in the acute phase of the disease within 3 weeks of onset by a dynamic pupillometer. Pupil diameters, latency, constriction velocity, constriction ratio and dilation velocity were noted. Regression analysis determined factors associated with pupillary response measurements, including visual acuity, color vision defect (HRR) and visual field defects ( Humphrey Visual Field Analyzer). 결과 : Pupillary constriction velocity and ratio were both significantly decreased in the ON and NAION group, but not in the LHON group. Pupil response latency was significantly delayed only in the ON group. In the ON group, pupillary constriction ratio significantly correlated with visual acuity (R2 = 0.701), color vision defect (RG: R2 = 0.783; BY: R2 = 0.573) and visual field defect (R2 = 0.558). Pupil response latency significantly correlated with visual acuity (R2 = 0.572) and color vision defect (RG: R2 = 0.385; BY: R2 = 0.394). 결론 : Pupillary constriction velocity and ratio were significantly decreased in ON and NAION, but latency was delayed only in patients with ON. Pupillary response measurements were not affected in patients with LHON. Dynamic pupillometry may be useful for differentiating acute optic neuropathies.
