망막동맥폐쇄 환자에서 retinal emboli의 특성과 망막혈관 재관류와의 관계

목적 : To analyze the characteristics of retinal emboli and its association with vascular reperfusion in retinal artery occlusion (RAO) 방법 : In this retrospective comparative case series, we analyzed emboli feature of 52 eyes diagnosed with acute CRAO (central retinal artery occlusion) and BRAO (branch retinal artery occlusion) with visible emboli,and analyzed vascular reperfusion of 29 eyes with RAO who have visible emboli responsible for perfusion blockade. At presentation, a detailed ophthalmic and medical history was obtained from all patients, and all underwent a comprehensive ophthalmic evaluation including fundus photography and fluorescein angiography. This evaluation was performed at each follow-up visits. 결과 : The total 52 eyes with RAO with visible emboli (21%) (CRAO=31, BRAO=21) exhibited various emboli features. Platelet-fibrin emboli (52%)were observed most commonly, and the highest rate of movement (100%) with the earliest time-point of movement (all P < 0.001). The 29 patients (movement=20,69% and no movement=9,31%) were compared vascular reperfusion states. Movement group ; platelet-fibrin(60%)and cholesterol(40%)emboli, early complete reperfusion was observed in 16 eyes(80%). No movement group ; cholesterol(67%) and calcific(33%) emboli exhibited late incomplete reperfusion in 6 eyes(67%). 결론 : Retinal emboli in RAO patients exhibit various characteristics, which affect emboli movement. Emboli movement may also affect vascular reperfusion and also other various mechanisms are concerned with vascular reperfusion even in case of no emboli movement. These mechanisms are regarded as a possible theory of preventing new vessel formation in RAO.
