갑상샘안병증 진단을 위한 외안근 크기의 기준

목적 : Extraocular muscle (EOM) enlargement in TAO is difficult to judge when it is mild and symmetric. As far as we know, there is no absolute cut-off value of EOM enlargement in TAO. A clear quantitative index would be helpful to define EOM enlargement in TAO. The purpose of this study is to determine the diagnostic cut-off value of EOM enlargement in patients with thyroid-associated ophthalmopathy (TAO). 방법 : We retrospectively reviewed the orbit magnetic resonance imaging (MRI) features of 66 orbits with TAO and compared findings with those of sex and age-matched controls. We measured the long diameters (LD), short diameters (SD) and areas of the four recti and superior oblique muscle on the coronal planes of the MRI. We also measured the long diameter of the four recti on the transverse and horizontal planes of the MRI. 결과 : There were differences between the TAO group and control group in the size of extraocular muscles except LD of lateral rectus muscle and LD of superior rectus muscle. The areas under the receiver operating characteristic curves (AUC) for diagnosing TAO was largest with the ‘(product of SD of four recti) / (LD of lateral rectus muscle)’ (AUC=0.931, p<0.0001). Using a cut-off value of ‘(product of SDs of four recti) / (LD of lateral rectus muscle)’ > 18.8, sensitivity was 86.36% and specificity was 86.36% in diagnosing TAO. 결론 : We established a diagnostic cut-off value of EOM enlargement in TAO using orbit MRI. The proposed formula is accurate and reliable for diagnosing EOM enlargement in TAO. In difficult cases to make a diagnosis of TAO, measuring the size of EOM and calculating supposed formula ‘(product of SD of four recti) / (LD of lateral rectus muscle)’ would be helpful.
