한국인의 결막내에서 발생한 MALT 림프종의 임상적 특징 및 방사선 치료 결과 분석

목적 : To evaluate clinical features and treatment outcome of radiotherapy for conjunctival MALT lymphoma in Korean patients 방법 : Ninety-four patients (22 male, 72 female; 44 unilateral, 50 bilateral) were diagnosed to have conjunctival lymphoma at Samsung Medical Center. All patients with conjunctival MALT lymphoma underwent initial staging work-up, received radiotherapy alone in local disease. We evaluated overall 5-year survival rate, local control, local relapse, systemic relapse, complications, and correlation between laterality and local relapse. 결과 : The series included 89 patients with MALT lymphoma including 2 patients with systemic involvement. All 87 MALT lymphoma patients received radiotherapy primarily : total dose of 26Gy in 78, 30Gy in 7, 36Gy in 1, and 40Gy in 1 patient. Overall survival rate, 5-year local control rate were 100%, 98.9% respectively. Eight cases showed local relapse after complete remission. The local relapse rate was highly associated with the unilaterality. Cataracts developed in 11(12.6%) patients with all lens shielded state during radiotherapy. 결론 : Conjunctival lymphoma in Korean patients takes a unique form of disease showing higher ratio of MALT lymphoma and lower systemic involvement at initial work-up and follow-up than in Western patients. Local radiation therapy (26Gy) could be a definite treatment for conjunctival MALT lymphoma. Especially in unilateral cases, long-term close monitoring would be necessary to check disease relapse. Focusing on complications, cataract inevitably occurs because of proximity to the conjunctival lesion, therefore, a clinical study is warranted for optimizing radiation dose.
