RGP 렌즈를 통한 Reis-Bucklers 각막이상증 환자의 corneal irregularity 교정

본문 : We hereby present a case of visual correction of Reis Bucklers corneal dystrophy combined with cornea ectasia by rigid gas-permeable lenses (RGP lenses). A 39-year-old woman was referred to us in July 30th 2015, complaining of low visual quality. She was diagnosed with Reis-Buckler dystrophy about 20 years ago. Her uncorrected vision was 20/200 in both eyes by Snellen chart. Her chief complaint was not improved by use of any glasses or soft contact lenses. She repeatedly received recommendation for surgical treatment, such as penetrating keratoplasty, by other hospitals. Under slit examination, moderate subepithelial opacities with amorphous, whitish deposits were noted in both eyes. The value of corneal simK on the topographic map was steep: 52.82D/flat: 46.17D on the right eye, and Steep: 47.27D/Flat: 45.18D on the left eye. The aberration map shows very poor results. In this case, we focused on correcting the corneal irregularity which we believed was the main cause of severe visual impairment by fitting the patient with RGP lenses. We decided to use the aspheric RGP lenses of Boston Envision®. The diameter of the lenses was 9.40mm. The Base curve was 7.60mm for the right eye, 7.80mm for the left eye with -4.50 spherical diopter in both eyes. With RGP contact lenses, her corrected visual acuity was 20/33 in both eyes. The value of corneal sim K was Steep: 40.81D/Flat: 40.23D on the right eye, and Steep: 39.38D/Flat: 39.20D on the left eye. Her visual aberration quality was improved dramatically despite severe corneal opacity. Through this case report, RGP lenses show significant possibility in non-invasive vision correction in various corneal dystrophies.
