목적 : The purpose of this study is to clarify the mechanism of developing DED by obesity, the study was performed by using in vivo mouse model as well as population based human cohort study. By the cohort based human study, we were tried to determine the relationship between DED and obesity, to investigate which demographic factors were correlated with DED severity. Also, using the db/db mouse based obesity mouse model, the pathologic changes of functional unit of tear production and the change of obesity related molecular markers, especially targeting neuropeptide Y (NPY), were investigated
방법 : Current analysis is an epidemiologic part of the Study of Environmental Eye Disease (SEED) which is a multidisciplinary project investigating diagnostic and therapeutic biomarkers for environmental eye diseases. We performed an ancillary survey on DED symptoms and signs for a subgroup of an established community-based cohort, the Korean Genome and Epidemiology Study (KoGES)-Kanghwa. The KoGES-Kangwha cohort enrolled 4,899 people living in Kanghwa Island, Incheon, South Korea between 2006 and 2011.
결과 : From the cohort study, BMI is correlated with ocular irritation symptoms and reduced tear secretion. And the level of Neuropeptide Y is significantly elevated and correlated with OSDI score and BMI. With the in vivo mouse model, fat cell infiltration and LG acinar cell loss was correlated with the duration of High fat diet fed. Interestingly, CD34+NPY+ cells were increased in BM and migrated into the DE induced lacrimal gland where the Y1R was elevated area. The loss of acinar and adipogenesis was synergistically increased by HFD and DE induction in db/db mouse model.
결론 : This study was investigated DED risk factor with using population based cohort study based model and compared the risk factors between both sexes, focused on the obesity parameters, chronic mental stress, and social relationship. The second is we provide the molecular target which may explain why high sugar and fat diet made patients has vulnerability of DE through the study of NPY system. Lastly, we hereby provide a novel mechanism of adipogenesis in LG, which showed corporative work between exocrine acinar and BM-derived CD34+ cells in DE stressed obese mice, enhancing the NPY and Y1R.
