건성안환자의 눈물샘에서 분비되는 IL-22의 기전과 역할에 대한 연구

목적 : Purpose: To investigate the expression and role of interleukin (IL)-22 in dry eye (DE). Design: Cross-sectional, case-control study; in vivo mouse experimental study. 방법 : Participants and specimens: 31 patients with DE and 18 healthy, age- and sex-matched controls, lacrimal glands (LGs) and eyeballs from DE-induced C57BL/6 and IL-22 knockout (KO) mice. Nano-liquid chromatography-tandem mass spectrometry and immunosorbent assay was used to quantify levels of cytokines in tear fluid of conjunctival sac (TF) and LG-secreting fluid (LF) from DE patients. All participants completed the ocular surface disease index (OSDI) questionnaire. The tear break-up time, Schirmer’s test, and corneal erosion score were assessed. Controlled environmental chamber was used 결과 : IL-17 levels were significantly higher in TF (3152.6±424.6 vs. 35.7±45.8 pg/ml) and LF (735.0±161.9 vs. <5 pg/ml) from DE patients than from control patients. Also, IL-22 levels were significantly higher in LF from DE patients (1847.1±404.7 vs. 759.3±230.5 pg/ml), and significantly lower in TF than in LF. IL-22 and IL-17 levels in tears significantly correlated with OSDI scores in DE patients. In C57BL/6 mice, IL-22 was expressed in normal LG acinar cells, and DE induced a 6.8-fold increase of IL-22 levels. 결론 : IL-22 and IL-17 levels in the ocular surface were reciprocally increased in tears from DE patients and correlated with patients’ symptom. In DE-induced mice, IL-22 was actively produced in LG acinar cells and secreted onto the ocular surface, where it prevented ocular inflammation. These data highlight the clinical implication that IL-22 system can be a promising therapeutic target against DE.
