목적 : To determine the factors predicting successful long-term outcome and the change in ocular alignment following surgery for acquired nonaccommodative esotropia (ANAET). 방법 : Medical records of 70 patients who underwent surgery for ANAET were reviewed. Surgical success was defined as an ocular alignment within 8 PD of orthophoria. The demographics, preoperative, and postoperative clinical data were compared between successful and unsuccessful surgical outcome groups. Correlations between the preoperative factors/initial postoperative angle of deviation and the amount of 3-year postoperative drift were investigated. The amount of 3-year change in ocular alignment was assessed in all patients and in each group according to the initial postoperative angle of deviation 결과 : Of the total 70 patients with ANAET, 43 patients showed long-term surgical success. Multiple logistic analysis showed that younger age at onset (OR=1.087; CI=1.018-1.160, p=0.012) and larger ocular misalignment at the first postoperative week (OR=1.680; CI=1.308-2.158, p<0.001) were significantly associated with unsuccessful surgical outcomes. The angle of deviation at the first postoperative week was the factor predicting the 3-year postoperative drift (r=0.394,p=0.001). The ocular alignment were change significantly in the initial undercorrections (9.0 ± 12.3 PD towards oesdeviation,P=0.011 결론 : Long-term surgical success was achieved in 61.4% of patients with ANAET. Later age at onset and smaller ocular misalignment at the first postoperative week were found to be associated with more favorable long-term surgical outcome. The 3-year postoperative drift had positive correlation with the angle of deviation at the first postoperative week.
