Re-activation of Emmetropization Process in Hyperopic Amblyopia?

목적 : To provide guidelines for prescribing spectacles for moderate to high hyperopic amblyopia and to show how emmetropization progress after comparing the results of amblyopia treatment and change of hyperopia according to the methods of spectacle correction. And to provide evidence of emmetropization progress in childhood with biometry data of the ocular components. 방법 : Hyperopic amblyopia children who had ≥+4.0 diopter (D) of spherical equivalent (SE) or more were included and who had >2.0 of astigmatism or with >2.0 of anisoemtropia were excluded. Patients were divided as full correction group fixed under correction group and post cycloplegic refraction (PCR) under correction group according to the amount of hyperopia correction. Change of initial hyperopia was compared between each group. Change of ocular refractive components such as cornea, anterior chamber depth, lens, vitreous chamber depth, and axial length were compared with A-scan biometry. 결과 : Seventy six eyes of 38 patients were enrolled in this study. Mean age of initial spectacle correction was 66.5 months and mean SE was +5.78 D. There was significant decrease of hyperopia and progression of emmetropization in under correction group. Initial hyperopia was decreased in under correction group by -0.28 D and -0.49 D at 6 months and 12 months respectively. Moreover, the amount of hyperopia under correction correlated with the amount of hyperopia reduction (P=0.010). During 12 months, total axial length was increased (P=0.011) and lens thickness was decreased significantly (P=0.012). 결론 : Under correction of moderate to high hyperopic amblyopia have beneficial effects for amblyopia treatment. And it also re-activate emmetropization process in childhood.
