한국인의 정상안압녹내장 중 low-teen과 high-teen 안압군에서 사상판경유압력차이 비교: 2009-2012년 국민건강영양조사.

목적 : To compare the estimated trans-lamina cribrosa pressure difference (TLCPD) and other systemic risk factors for glaucoma development between low-teen and high-teen intraocular pressure (IOP) patients with normal tension glaucoma (NTG) using a population-based study design. 방법 : A total of 9,177 adults (≥40 years of age) who participated in the Korean National Health and Nutrition Examination Survey (KNHANES) from 2009 to 2012 were included. Using a previously developed formula, cerebrospinal fluid pressure (CSFP) in mmHg was estimated. The NTG subjects were divided into two groups according to IOP level: low-teen open-angle glaucoma (OAG) (IOP ≤ 15 mmHg) and high-teen OAG (15 mmHg < IOP ≤ 21 mmHg) groups. The association between TLCPD and the prevalence of glaucoma was assessed in the low- and high-teen IOP groups. 결과 : In normal population (n = 8,499), mean estimated CSFP was 11.50±3.08 mmHg and mean TLCPD 3.31±3.92 mmHg. Significantly higher TLCPD (7.19±3.83 mmHg) was found in the high-teen OAG compared with the control (p<0.001). On the other hand, there was no significant difference in TLCPD between normal and low-teen OAG (p>0.05). Logistic regression analysis revealed TLCPD was strongly associated with the prevalence of glaucoma in high-teen IOP group but not low-teen IOP group. Instead, the presence of hypertension was significantly associated with the prevalence of glaucoma in low-teen IOP group. 결론 : TLCPD was significantly associated with the development of glaucoma in subjects with high-teen IOP, but not those with low-teen IOP, in whom hypertension may exert a greater influence. This study suggests that the underlying mechanisms differ between low-teen and high-teen OAG patients.
