망막박리 또는 망막열공을 동반한 심한 유리체출혈에서 유리체절제술의 시점에 대한 고찰

목적 : To compare the visual outcomes of early intervention versus delayed intervention in the setting of fundus-obscuring vitreous hemorrhage presumed to be due to breaks or detachment of retina 방법 : All eyes with a fundus-obscuring vitreous hemorrhage defined as vision of 20/400 or worse, from 2012 - 2014, were evaluated. Patients were divided into two groups: "Early Intervention (vitrectomy performed within 1 week of diagnosis)" and "Delayed Intervention (vitrectomy performed after 1 week of diagnosis)". Post operative diagnoses were evaluated, and eyes that were diagnosed with vitreous hemorrhage due to causes other than retinal tears or detachment were excluded. Best corrected visual acuity at 2 months and 4 months after the surgery were uased as the main outcome measure, 결과 : 75 eyes met inclusion criteria with a mean follow-up of 120 days. Initial BCVA was logMAR 2.218 and final BCVA was 0.318. 49 patients were in the "Early Intervention" group and 26 patients were included in the "Delayed intervention" group. The two groups showed no significant difference in the extent of improvement of vision (p<0.05). No patients showed progression to a macula-off retinal detachment in both groups. 결론 : Early surgical intervention results showed similar visual outcomes compared to a delayed surgical intervention. Other factors that may influence clincal outcomes of VH should be further investigated.
