To report a case of irreversible damage on central fovea in Indocyanine green (ICG) assisted internal limiting membrane (ILM) peeling.
Case Report
A 77 year-old male with ERM on his right eye received pars planar vitrectomy followed by ILM peeling. After ILM peeling, ICG dye was applied to identify remnant ILM tissue. Immediately, ILM-peeled macular area was colored greenly with oval shape and the coloration persisted despite several times of washing with balanced salt solution (BSS). The patient was asked to visit our clinic after 1 week, 1 month, 3 month, 6 month and 1 year after the operation. Fundus photography and spectral domain optical coherence tomography (SD-OCT) showed obvious macular coloration and outer retinal disruption respectively. Visual acuity which was 20/100 preoperatively, has dropped off around 20/500 without showing remarkable recovery along with the clinical course.
Conclusion
ICG dye should be used with caution and usage of ICG onto bare retina should be avoided.
