To report the long term-followed 2 cases of expulsive suprachoroidal hemorrhage which intervened by timely silicone oil tamponade and yielded favorable clinical outcome.
Case Summary
Case1 : A 76-year-old male receiving phacoemulsification on his left eye cataract was reffered for suprachoroidal hemorrhage detected intraoperatively. Anterior chamber was totally flattened and iris was pushed back and trapped in surgical main wound. Silicone oil tamponade was performed to flatten embossed retina without inserting intraocular lens (IOL). A month after, planned IOL insertion was performed with in the bag style. Best corrected visual acuity measured 11 months after the IOL insertion was 0.5 and no other complication was observed.
Case2 : A 57-year-old male who received cataract surgery 20 years ago and referred for IOL dislocation on his left eye. During IOL scleral fixation after routine pars plana vitrectomy, suprachoroidal hemorrhage was detected. Noticing PFCL (perflouorocarbon liquid) use not being successful in flattening retina, rapid silicone oil tamponade and wound closure was followed by. Existing IOL removal and new IOL scleral fixation was performed 3 months after. Best corrected visual acuity measured 13 months after the new IOL scleral fixation was 1.0 and no other complication was observed.
Conclusion
Rapid identification and prompt decision to use silicone oil tamponade can avoid devastating consequences of expulsive suprachoroidal hemorrhage and bring about favorable long-term clinical outcome.
