망막전막이 있는 환자에서 경구 스테로이드가 백내장 수술 후 황반부종에 미치는 영향

목적 : To investigate the effect of oral steroid on pseudophakic cystoid macular edema (CME) in patients with epiretinal membrabne (ERM) after phacoemulsification and intraocular lens (IOL) implantation 방법 : A retrospective chart review was performed on subjects who underwent uneventful phacoemulsification and IOL implantation (n=1349), and changes in macular thickness and CME were compared in patients with preexisting ERM (n=81) with post-operative oral steroid use (n=45) and without oral steroid (n=36). Topical NSAIDs were administered in all patients. Definite or probable CME were defined by presence of changes in retinal contour with or without cystoid changes. Presence of change in central macular thickness of more than three standard deviations (SD) (≥ 70.17µm) was defined as possible CME. 결과 : The incidence of definite, probable and possible CME were 2.22% (n=1), 4.44% (n=2), 6.67% (n=3) with usage of steroid and 2.70% (n=1), 5.40% (n=2), 5.40% (n=2) without steroid, which was insignificant, respectively. Changes of central foveal thickness, 3mm and 6mm zone of the macula were 21.05 µm, 40.02 µm, 34.43 µm with usage of steroid and 21.03µm , 41.78 µm, 40.19 µm (p < 0.05) without steroid. Thickness was significantly decreased in 6 mm zone with usage of the steroid. 결론 : This study suggests that usage of oral steroid affect changes of retinal thickness in patients with preexisting ERM after phacoemulsification and IOL implantation, although it may not prevent clinically significant CME.
