녹내장성 구조적 변화 진단 시 스펙트럼영역 빛간섭단층촬영의 보조적 역할

목적 : To investigate the additive role of spectral-domain optical coherence tomography (SD-OCT) for structural diagnosis in glaucoma. 방법 : One hundred and nine eyes of 109 healthy individuals and 151 eyes of 151 glaucoma patients were included. Four structural diagnostic examination sets were prepared using stereo optic disc photography (SDP), red-free retinal nerve fiber layer photography (RNFLP), and SD-OCT: (1) SDP (S), (2) SDP and SD-OCT (SO), (3) SDP and RNFLP (SR), and (4) SDP, RNFLP, and SD-OCT (SRO). Five glaucoma specialists were instructed to classify subject as normal or glaucoma using each set with 1-month interval. The interobserver agreement of specialists and the additive diagnostic effect of SD-OCT were evaluated. 결과 : Five glaucoma specialists showed an excellent level of interobserver agreement on the diagnostic assessment based on 4 sets. In each glaucoma specialist, the diagnostic performance increased with additional SD-OCT to SDP, although showing difference in significance. Addition of SD-OCT to SDP showed about 2-folds increase in the overall diagnostic performance of all specialists. The diagnostic performance increased with the increasing number of structural examinations, SRO showing the best performance about 7-folds higher than S-only. 결론 : SD-OCT significantly enhanced the glaucoma-diagnostic accuracy of glaucoma specialists’ assessments. Even for glaucoma specialists, SD-OCT has an additive value for improvement in the glaucomatous structural assessment.
