본문 : Here we report a case of complete occlusion of anterior capsulorhexis, after uncomplicated cataract surgery, treated with non-invasive treatment. A 69-year-old woman visited our clinic with decreased visual acuity of her right eye for 2 months. Her ocular history was positive for Laser iridotomy on her right eye, 3 years ago, due to acute angle closure glaucoma. With the diagnosis of cataract, the patient underwent phacoemulsification and posterior chamber IOL implantation on the right eye. Although zonular weakness was observed, there was no intraoperative complication. 1 month later, the patients best corrected visual acuity (BCVA) was 20/20 with no other remarkable findings. The patient presented again because of blurred vision in the right eye. Slit lamp examination revealed total occlusion of anterior capsulorhexis. Circular anterior capsulotomy was performed by neodymium:YAG (Nd:YAG) laser. After the laser treatment, steroid were used both systemically and topically. One week later, BCVA in her right eye was 20/20 with no other remarkable findings including inflammation. Several cases are reported about surgical removal of membrane and radial Nd:YAG anterior capsulotomy for complete occlusion of anterior capsulorhexis, which is an uncommon complication after cataract surgery. In this case, as the non-invasive procedure, circular Nd:YAG laser anterior capsulotomy was used for treating complete occlusion of anterior capsulorhexis. In conclusion, Nd:YAG laser anterior capsulotomy can be used as the first line treatment in similar cases. |
