중합효소 연쇄반응(PCR)을 이용한 한국인 포도막염의 임상 양상 분석

목적 : The present study was designed to clarify polymerase chain reaction (PCR) analysis of aqueous humor and vitreous as a test for uveitis in Korea. 방법 : Patients who underwent PCR testing of ocular fluid (aqueous humor and vitreous) for uveitis at Seoul National University Hospital between January 1, 2005 and December 31, 2014, were enrolled. Total one hundred ninety-eight patients were selected and their charts were reviewed. Vitreous specimens were gained by passing a 23 gauge needle or vitreous cutter through the pars plana. Anterior chamber aqueous samples were obtained using a 30 gauge needle on a 1cc syringe. Baseline characteristics of PCR were identified. 결과 : There was a male predominance, and the mean age was 56.2 years. Herpetic keratouveitis(66.7%) and CMV retinitis(60%) were the diseases with the most frequently positive results. The sensitivity was 59% and the specificity was 73% in aqueous humor PCR. The sensitivity therefore was 59% and the specificity was 73% in aqueous humor PCR. As a consequence of aqueous analysis, change of treatment was necessary in 31.0% cases. No complications of specimen acquisition were reported. 결론 : In summary, PCR of ocular fluids can be a safe, easily performed diagnostic test, especially it can be useful if herpetic keratouveitis or CMV retinitis is suspected. But this study reveals that PCR testing might not have been sensitive enough to detect all pathogens.
