시신경 유두의 혈액 관류에 대한 해부학적 고찰

목적 : To investigate the microvascular anatomy of optic nerve head (ONH) perfusion using indocyanine angiography (ICGA). 방법 : Thirty-two eyes from 32 patients with primary open-angle glaucoma were included. ICGA centered on the optic disc was performed using a scanning laser ophthalmoscope equipped with an eye tracker. The microvessels directly responsible for blood supply to the ONH were sought. The temporal relationship between when the dye first appeared in the ONH and in the retinal vessels, parapapillary large choroidal vessels, and cilioretinal artery (in cases with a cilioretinal artery) were determined. 결과 : ONH perfusion was achieved in a sectoral fashion. The time interval between when the dye first appeared in the ONH and the peak ONH filling was 9.48±2.01 s. The temporal relationship between when dye first appeared in the ONH and in the other vessels varied among the patients. However, the time when the dye first appeared in the ONH uniformly coincided with the appearance of microvessels between the parapapillary choroid and the ONH. The ONH was not perfused until the visualization of those microvessels. 결론 : These data suggest that ONH is supplied mainly by the centripetal small vessels that originate from the parapapillary choroid.
