Hyperaspheric, Mildaspheric Corneal wavefront-guied ablation profile 및 Mildaspheric ablation Aberration-free profile 에 따른 근시 교정 레이저 각막절제술의 임상결과 비교

목적 : To compare the efficacy, predictability, stability and HOAs(Higher-order aberrations) outcomes between Hyperaspheric(Group I), Mildaspheric corneal wavefront-guided(CWFG)(Group II) and Mildaspheric aberration free(Ab-free)(Group III) aberration ablation profile in photorefractive keratectomy(PRK). 방법 : The study enrolled 165 patients (300 eyes) willing to undergo PRK from November 2014 to February 2015 at Eyereum Eye Clinic, Seoul, Korea. Patients were randomly assigned to three groups each to undergo PRK treatment with Mildaspheric Ab-Free, Mildaspheric CWFG or Hyperaspheric CWFG profile and treated PRK using the AMARIS excimer laser (SCHWIND eye-tech-solutions GmbH, Kleinostheim, Germany). Data on the manifest refraction, uncorrected visual acuity, best-corrected visual acuity(BCVA), spherical equivalent and higher-order aberrations were collected preoperatively and at the 1-, 3 mont 결과 : All groups had improvement of UCVA at postoperative(PO) 3 months(P<0.001) and there was no significant difference between three groups(P=0.12). All patients reached UCVA of 20/25 or better at 3 months. At PO 3months, total RMS in Hyperaspheric CWFG group was significantly lower than other groups(P=0.049, 0.045). The change of total RMS between preoperative and PO 3 months showed no difference between the three groups(P=0.14). Hyperaspheric CWFG group induced spherical aberration less than other two groups (P=0.03 and 0.02). All ablation profile didn't cause the changes in coma(P=0.25). 결론 : A difference in uncorrected visual acuity safety between eyes undergoing PRK with Hyperaspheric, Mildaspheric corneal wavefront-guided(CWFG) and Mildaspheric aberration free(Ab-free) aberration ablation profile at 3 months could not be identified. In HOAs, Hyperaspheric CWFG profile less induced total RMS, spherical aberrations than other two profiles, but there was no difference in coma between three ablation profiles.
