목적 : To compare clinical, angiographic and optical coherence tomographic characteristics between eyes with acute Vogt-Koyanagi-Harada (VKH) disease and eyes with acute bilateral central serous chorioretinopathy (CSC) and to demonstrate features distinguishing two disease in confusing cases.
방법 : The medical records of 35 patients with VKH disease and 25 patients with bilateral CSC were retrospectively reviewed. Slit lamp biomicroscopy, ophthalmoscopy, fundus photography, fluorescein angiography (FA), indocyanine green angiography (ICGA), and spectral-domain optical coherence tomography (OCT) were compared between two diseases.
결과 : Five of 35 patients (10 of 70 eyes, 14.3%) with bilateral VKH disease were initially misdiagnosed as CSC and six of 25 patients (12 of 50 eyes, 24%) with bilateral CSC were initially misdiagnosed as VKH disease. Pigment epithelial detachment (PED) on OCT could be good distinguishing feature in CSC, because PED was not observed in any cases of VKH disease. On the other hand, optic disc hyperemia and disc leakage on FA could be good distinguishing points of VKH, because these manifestations rarely show in CSC eyes.
결론 : The most reliable features to distinguish acute VKH disease and acute bilateral CSC could be disc hyperemia in VKH eyes and PED on OCT in CSC eyes.
