목적 : The aim of this study is to evaluate the reliability and accuracy of AS-OCT in the measurement of the lateral rectus (LR) insertion distance, to investigate the longitudinal change of LR muscle insertion after strabismus surgery, and to determine the effect of insertion movement on the angle of deviation after strabismus surgery.
방법 : We recruited 30 patients who underwent primary unilateral or bilateral LR muscle recession surgery with normal anterior segment structures. An AS-OCT scan of the LR muscle was performed every visits. Preoperative limbus-LR insertion distance was compared with intraoperative surgical caliper measurement of LR insertion using intraclass correlation coefficient analysis (ICC). Data on gender, age, degree of deviation (prism diopter), and spur-LR insertion distance using AS-OCT were collected at postoperative months 1,3 6.
결과 : Excellent agreement was observed for the measurement of limbus-LR insertion using ICC analysis (ICC=0.71). There was no significant difference in spur-LR insertion and the angle of deviation between each postoperative visit. However, the change of length in the spur-LR insertion, as measured by AS-OCT, was significantly related to the amount of postoperative deviation. When spur-LR insertion increases by approximately 1 mm, the angle of deviation decreases at a rate of 0.94 PD (p<0.01).
결론 : In conclusion, AS-OCT is a useful instrument to evaluate the insertion of extraocular muscle (EOM) before strabismus surgery, and also has potential use in understanding postoperative changes in EOM after tissue swelling subsides.
