눈꺼풀 수술 시 2% 리도케인, 2% 잘로케인, 2% 잘로케인-부피파케인 혼합 마취제의 통증의 전향적 비교 연구

목적 : Xylocaine contains 2% Lidocaine and 1:100,000 epinephrine for a hemostasis benefit. Xylocaine-bupivacaine mixture was used to expect the combination of fast-onset anesthetic effect of xylocaine and long-duration anesthetic effect of bupivacaine. We aim to compare the pain score of 2% Lidocaine, 2% Xylocaine, and Xylocaine-bupivacaine mixture for eyelid surgeries. 방법 : This was a double-blindrandomized prospective and comparative study.120 consecutive patients (43 males, 77 females, mean age=47.2 ± 21.2) who underwentbilateral eyelid surgery under subcutaneous anesthesiawere asked their pain score on each eyeduring the first anesthesia injection. Each patient wasrandomly assigned to receive one of the three anesthetic agents; 2% Lidocaine, 2% Xylocaine, and 2% Xylocaine-bupivacainemixture in one eyelid, and one of the remaining two agents in the other. 결과 : The pH of 2% lidocaine, 2% Xylocaine and Xylocaine-bupivacaine mixture were about 6.23 ± 0.21, 4.21 ± 0.37,and 3.87 ± 0.19, respectively.The pain score of each substance (2% Lidocaine, 2% Xylocaine, and 2% Xylocaine-bupivacaine mixture) was4.3 ± 1.8, 5.1.± 1.8, and 5.7 ± 1.9, respectively.The 2% Xylocaine produced a statistically significant higher pain score than 2% lidocaine (p=0.044, generalized estimating equation [GEE] methods).Xylocaine-bupivacaine mixture also showed a significant higher pain score compared to 2% lidocaine (p=0.003, GEE methods). 결론 : Epinephrine containing definitelyseems to increase subjective pain score. Compared to 2% Xylocaine,Xylocaine-bupivacaine mixture does not affect on subjective symptom and change of pH.
