레이저 포인터 조사 후 발생한 황반병증 1예

본문 : Purpose: To report a case of successful treating maculopathy after exposure to a handheld laser pointer beam. Case summary: A-15-year-old patient visited our clinic complaining of visual disturbance on the left eye 5 days ago after exposure to a handheld laser pointer with 532 nm wavelength green beam for 5 seconds. His best corrected visual acuity was 0.4 in the left eye. On fundus examination, a yellowish retinal scar was observed at forveal area. The optical coherence tomography showed inner segment/outer segment line disruption and retinal pigment epithelial injury related to laser pointer exposure. We started occlusion therapy on his left eye and oral prednisolone, antioxidant, and capillary stabilizing agent treatment for 2 weeks. The best corrected visual acuity was 0.7 at 2 weeks and was 1.0 at 1 month in the left eye after treatment and subjective clinical symptoms could also be improved. But scar change was remained in the retinal pigment epithelium layer of the macular region of his left eye while inner segment/outer segment line was restoration by optical coherence tomography. Conclusion: It could lead a maculopathy after exposure to handheld laser pointer. In case of maculopathy with 532um wavelength green beam, the occlusion therapy, oral prednisolone, antioxidant, and capillary stabilizing agent might be helpful for recovery of visual acuity and restoration of inner segment/outer segment line.
