Management of congenital elevation deficiency due to congenital third nerve palsy and monocular elevation deficiency

목적 : To document the presentation and management of congenital III nerve palsy (CN3) and monocular elevation deficiency (NED) at a single clinic over a 14-year period. 방법 : Retrospective review of clinical case notes were done of all patients who had surgical treatment for either CN3 or MED between 1999 to 2012 was done. Pre- and post-surgical data were documented and analysed. 결과 : A total of 5 congenital III nerve palsy and 15 monocular elevation deficiency patients were identified. Mean preoperative vertical deviation was 22.4 prism diopters (PD) and horizontal deviation was 46 PD for CN3 group and 33.9 PD and 2.4PD for MED group (P=0.02). Mean follow-up time was 27 months for both groups. Postoperative vertical deviation was 4.2 PD and horizontal 0PD for CN3 group and 4.5 PD and 1.47 PD for MED group. At last follow up amblyopia remained in 2 patients (40%) in CN3 group and 5 patients (33%) in MED group. 결론 : Superficially, CN3 and MED may appear similar, both frequently having ptosis and hypotropia as features. CN3 more frequently requires surgery for exotropia as well as surgery for hypotropia and MED more often requires surgery just for hypotropia. The ptosis surgery was similar for either diagnosis in this study (P=0.61). Significant cosmetic improvement , as well as excellent visual acuity outcomes can be achieved.
