목적 : To investigate how postural changes affect the intraocular pressure (IOP) in silicone oil (SO)-filled eyes.
방법 : Seventeen patients with the SO-filled eyes participated. IOP was measured using an electronic tonometer in each of the following positions: [sitting(T1)→ supine(T2) →5 minutes after SO-filled eye dependent lateral decubitus position(LDP)(T3) → 30 minutes after SO-filled eye dependent LDP(T4) → re-supine 1(T5) → 5 minutes after fellow eye dependent LDP(T6) →30 minutes after fellow eye dependent LDP(T7) →re-supine 2(T8)]. The IOP of SO-filled eyes and fellow eyes in each position were compared.
결과 : The mean IOP of SO-filled eyes were higher than fellow eyes in the sitting(T1), supine(T2), SO-dependent LDP positions(T3, T4, T5) and re-supine 2 position(T8) (P<0.05, respectively). The mean IOP of the dependent eye increased at 30 minutes after changing from the supine to the dependent LDP in both SO-filled eyes and fellow eyes (P=0.001 and 0.008). The mean IOP of the dependent eye decreased at 10 minutes after changing from the dependent LDP to the re-supine position in both SO-filled eyes and fellow eyes (P=0.011 and 0.007). The IOP elevation by postural change from the supine position to the LDP was greater in SO-filled eyes than fellow eyes (P=0.016) and the decrease of the IOP from the LDP to the supine position was greater in SO-filled eyes than fellow eyes (P=0.012).
결론 : The mean IOP of the SO-filled eyes showed greater change by postural change from the supine to the LDP and also in inverse, compared with the fellow eyes. Patients with long-term SO endotamponade for severe vitreoretinal diseases have to be taken into consideration to reveal the SO globules by the gonioscopy. In such cases, SO-filled eye dependent positions may be prudent for patients with a history of glaucoma. Our study could be informative to the further investigation of the effect of LDP on IOP in SO-filled eyes with glaucoma.
