Causes and Prognosis of acute onset Unknown origin vitreous hemorrhage

목적 : To analyze the causes and prognosis of acute onset unknown origin vitreous hemorrhage 방법 : Retrospective review was conducted to the patients who were underwent vitrectomy for acute onset undiagnosed vitreous hemorrhage (VH) from January 1997 through October 2013. Postoperative diagnosis was analyzed in each patients. Baseline characteristics such as best corrected visual acuity (BCVA), follow-up duration, unaffected fellow eye findings were compared between postoperative diagnosis. And by using bright flash electroretinogram (bfERG), relationship between bfERG parameters and postoperative BCVA. 결과 : 169 eyes with undiagnosed VH were included this study. Among them, retinal vein occlusion(RVO), retinal break and Age related macular degeneration (AMD) were 43.8%, 29.6%, 12.4%, respectively. In age under 60, retinal break was the most common cause of undiagnosed VH. And In age over 60, RVO was the most common cause of undiagnosed VH. The proportion of AMD as a causes of undiagnosed VH was increased with age. Preoperative and postoperative BCVA of AMD were significantly worse than others. And there was significant positive correlation between a&b wave amplitude in bfERG and postoperative BCVA 결론 : RVO, retinal break and AMD were the most common causes of acute onset unknown origin vitreous hemorrhage. And the most common cause of unknown VH was changed with age. Except AMD, the visual prognosis of unknown origin vitreous hemorrhage was relatively good. Older age patients with poor preoperative BCVA and AMD in fellow eye had higher change of AMD as the causes of acute onset unknown origin vitreous hemorrhage.
